Benefit Summary
Medina General Hospital continues to invest in the resources necessary to deliver high-quality health care to those we serve by providing a competitive and diverse benefit package. Through MGH Select , you have a range of benefit choices that include: · Health Care coverage, including prescription drug coverage
· Dental Care coverage
· Vision Care coverage
· Life Insurance

o Dependent Life
· Health Care Reimbursement Account (HCRA)
· Dependent Care Reimbursement Account (DCRA)
· Short-Term Disability
· Long-Term Disability
· Long-Term Care (for example, nursing home care)
Although MGH pays most of the cost of your benefits, you share in the cost. Your portion of the cost is paid through payroll deductions with tax-free dollars whenever permissible by law.
Medical Care Coverage
You can choose between two medical coverage options – each offering certain advantages. With both plans, you receive the highest level of benefits when you access care from MGH and Medina Physician Hospital Organization physicians (http://medinapho.com/index.htm). You also receive discounted services through the Medical Mutual of Ohio (MMO) network. Out-of-network services are covered by the plan but you may pay more for these services. Prescription drug coverage is also included with both medical plans at no additional cost. You may purchase prescriptions at a pharmacy and you can save money filling your maintenance prescriptions through the mail order program. For more details on the Prescription Plan, click here.
You can choose between two dental coverage options – each offering certain advantages. With both plans, you are free to use any dentist for services and treatments. For more details on the Dental Plans, click on the above link.
The VisionCare Plan helps cover the cost of eye exams and other vision expenses. You can elect vision coverage even if you do not participate in an MGH medical or dental plan. For more details on the Vision Plan, click on the above link.
Flexible Spending Accounts
You can set aside tax-free dollars – up to $5,000 a year – to reimburse yourself for out-of-pocket expenses, (such as deductibles and office visit co-pays) and other eligible expenses, (such as orthodontia and certain over-the-counter products) that may not be covered by insurance.
You can set aside tax-free dollars – up to $5,000 a year (or $2,500 a year if you are married filing separately) – to reimburse yourself for Dependent Care in your home, someone else’s home, or in a child care or dependent care facility. MGH pays the cost of providing you with coverage of 1 x base salary or a minimum of $10,000. You have the ability to buy additional coverage equal to 1x or 2x base salary.* If you die, your beneficiary will receive the life insurance amount you have chosen.

MGH offers you the ability to purchase Group Term Dependent Life Insurance for your spouse and/or dependent children. For more details on the Life Insurance Plans, click on the above link.
MGH provides Short-Term Disability (STD) coverage for all full-time and part-time employees with more than one year of service. STD benefits replace a part of your earnings if you are totally disabled for a short period of time because of a non-work related injury or illness. Under this plan, your benefit will be 60% of your weekly earnings, up to a maximum of $1,500 per week. STD benefits begin after a 7-day elimination period once you have been disabled. Benefits will continue for up to 12 weeks or as long as you continue to be disabled as defined by the plan. For more details on the Short-Term Disability Plan, click on the above link.
In the event that you are totally disabled and unable to work after 12 weeks of disability, Long-Term Disability (LTD) coverage can provide you with income. Full-time employees, with more than one year of service, are eligible for LTD coverage equal to 50% of pay, up to a maximum of $5,000 per month (after a 90-day waiting period). For full-time employees, MGH provides this coverage at no cost to you. Full-time employees can purchase additional LTD coverage equal to 60% of pay, up to a maximum of $6,000 per month (after a 90-day waiting period). Part-time employees, with more than one year of service, can purchase LTD coverage equal to 60% of pay, up to a maximum of $6,000 per month (after a 90-day waiting period). For more details on the part-time LTD plan, click on the above part-time link.
Long-Term Care Coverage helps pay nursing home or professional home care expenses in the event you become incapacitated. You can also purchase Long-Term Care Coverage** that provides:
· Benefits for home care provided by a friend or relative
· Coverage for your spouse, parents, and/or other specified relatives
· Inflation protection
· Higher benefit amounts
Keep in mind that you can continue your coverage if you leave MGH.

When can I change my coverage choices?
IRS regulations require your elections for Medical, Dental, Vision, and Flexible Spending Accounts to remain in effect until December 31, 2008. However, if you have a “qualified status change,” you can make certain changes if you contact Human Resources within 31 days of the event and provide written notice of the change.
Qualified Status Changes include:
· Your marriage or divorce
· The death of your spouse or dependent
· The birth or adoption of your child
· A change in your child’s eligibility status
· A change in the employment status of you/your spouse (for example, you change to full-time and become eligible for benefits, or your spouse loses coverage because he/she changes to part-time)
· Your or your spouse’s unpaid leave of absence
· A significant change in the health care coverage provided by your spouse’s employer
Does my age affect my MGH Select contributions?
Age does not effect your Medical, Dental, Vision, or LTD contributions.
Your contributions for Group Term Life Insurance (at 1x or 2x base salary) will increase as your age increases.
Your contribution for Long-Term Care is based on your age when coverage begins. Contributions are higher for older participants. Once enrolled, your contributions will not change, unless the insurance company increases contributions for everyone in your age bracket.
How will pay increases impact my coverage?
Pay increases affect your Group Term Life Insurance and Disability Coverage. If your pay increases, the benefit paid will be higher, so your contribution amount will be higher too.
